(Re) Scheduling Request Form

To ensure availability and respect for scheduled time slots:

Clients have (1) month from the missed session to reschedule, see the "Common Questions" tab.

Rescheduling Fee: $10.00. -- If arranged for a group session, all members of the group must pay the rescheduling fee in order to attend the rescheduled session.

The rescheduling fee must be paid in advance before a new calender invitation link can be sent for you to reschedule.

To request a reschedule, please complete the "Reschedule Request Form" and submit. -- If for a group, each group member must submit a request form.

Once your rescheduling status has been verified, you will either receive:
- A payment link -&/or-
- A calendar link, all group sessions will be arranged by a representative.

Client may request one (1) complimentary reschedule or waiver, at company discretion.
