(Re) Scheduling Request Form

To ensure availability and respect for scheduled time slots:

• If applicable, clients have (30) days from the missed session, see service agreement for more details.

• To request a reschedule, please complete the "Reschedule Request Form" and submit. -- If for a group, each group member must submit a request form. 

• Once your rescheduling status has been verified, you will either receive:
  • A payment link   -&/or-
  • A calendar link, all group sessions will be arranged by a representative. 

• If applicable, cient may request one (1) complimentary reschedule or waiver, a company discretion.
