Instructions:

Please see your terms & conditions agreement for provisions regarding the scheduling process. In most cases, the packages are valid for one year.

Within (48) hours of this submission, you should receive a service addendum email, sent to you for your signature.

Upon signing the service addendum, you will receive a calendar invitation to schedule your upcoming session.

If your request is denied, you will receive an email informing you of the decision and the reason for the denial.
